Use the product name when you ask.
Ask whether the named medicine is on the plan’s formulary. A formulary names the medicines your insurance plan pays for.
Find out if your plan needs prior authorization. That means the plan must agree in advance before it pays.
Ask what information the plan needs from the health professional.
Check the visit and the tests too.
Put separate questions to the plan: is the online visit included? Is the named health professional covered? Where can the lab order be filled under the plan?
Ask what you would owe for each part. Ask who can give a written estimate. Keep the answer with the date and the name of the plan you checked.
If you hear that a visit accepts insurance, ask whether that also applies to the tests and the medicine.
Compare paying yourself with using insurance.
Ask what happens if the plan declines payment. Would you be charged automatically, or would you be asked to agree before paying yourself?
Before paying without insurance, ask your insurer: would this payment count toward the amount I must pay before insurance starts paying? Ask what your share of this bill would be if you used insurance instead.
